– An application in the form prescribed by the Division
– Proof that the applicant has complied with the requirements for education, training, experience, and certification required for each

Nevada has statute, NRS 656 A: which requires any interpreter working in the state of NV to be registered with ADSD (Aging and Disability Services Division)
