– Certification of graduation from High School or equivalency
– Proof of an accepted certification
– The required fee

Provisional Licensure of the following:

– Proof of an accepted certification
– Proof that the applicant has completed and is awaiting results of a test required for acceptable certification

License Annual renewal required – the license expires on 12/31 of each year.
License renewal may take place during the sixty (60) days before date of expiration
Provisional expires twelve (12) months from the date of issuance
